Position Summary: Under the direction of the Health Director, the JPC's main responsibility will be based on the client and family-centered care model and in accordance with the vision and values of the community. This position is responsible for providing service coordination to the children and families of Fishing Lake First Nation.

Duties and responsibilities

1. Outreach and community engagement

   1. Build relationships within the community and collaborate with those providing existing community-based services

2. Intake, assessment, and coordination

   1. Assisting the existing Service coordinator with administrative and financial tasks

   2. Assisting with applications and their current statuses

   3. Scheduling appointments

   4. Contacting families, vendors, or professionals

3. Case Management

   1. Documentation of case files

   2. Maintaining records and invoices on file for audit purposes

   3. Database tracking system

   4. Hard copy filing system

Qualifications

• Secondary school diploma or equivalent education required (Grade 12)

• Some demonstrated office skills, problem-solving skills

• Valid Class 5 Driver’s license

• Valid Criminal Record Check & Vulnerable Sectors check
